A showcase exhibiting how two pioneering services work perfectly together to enable and empower stroke survivors in both prolonging life and improving quality of life.
Adding Years to Life: Health Promotion Service, Hull
This service works to enable and empower people to take control of their lives and prevent either secondary or primary stroke through group work and discussion involving exercise, lifestyle analysis and self-learning (including the presentation of their own research findings). Peer support and the involvement of volunteers are integral to the service, giving clients the opportunity to help not only themselves but others as well.
Adding Life to Years: Communication Support, Trafford
Individual needs are at the heart of this very special service in Trafford that enables stroke survivors with devastating communication disabilities to regain their confidence and independence, and challenge social isolation. Personal attention is key in setting and achieving short-term goals with clients (even those with severe impairments), the provision of support and training for carers, and close joint working with Speech and Language Therapists. Importantly, peer support and volunteer-led activity aim to empower all clients, not simply improving quality of life but truly transforming it.
